How to build custom reports by type in FORWARD Direct using the “Add report” window.

When Should I Use This?


Use Building Custom Reports when you need to:

  • Add a report to the portal from the “Add report” window as a Process-Admin Team Member

  • Pull PII (Personally Identifiable Information) for submitted applications (for secure workflows like auditing). 

  • Review applications that were started but not submitted

  • Choose a payment schedule report, including options labeled NON-PII or PII

  • Track approvals and throughput with Approval Report

  • Export or monitor general application submission data with Application Data Report.


Step-by-Step Instructions


  1. Go to Manage Program

  2. Select Reports

  3. Select “Add report. 

  4. In the dropdown, go to SELECT THE REPORT

  5. Choose the report you need (examples below).

    • Application PII Data Report: detailed PII data for submitted applications; used for auditing or secure data handling. 

    • Basic Report - Programs: summarized view of applications/activities in self-serve programs; includes status, dates, and applicant details. 

    • Forms non-submitted apps: shows how many applicants started an application but did not submit. 

    • Payment schedule - overall NON-PII report: payment schedule report labeled NON-PII. 

    • Payment schedule - overall PII report: combines payment scheduling data with PII elements; sensitive and used by finance or compliance teams.

    • Approval Report: shows approval statuses and related metrics; helps track decisions and throughput. 

    • Application Data Report: general report capturing key data fields from submissions; useful for performance monitoring, exports, or analytics. 

  6. Go to SELECT THE ROLE (see User Roles)

  7. Select the correct role for the report you’re adding (the guide notes PII-Viewer is the most common).


Common Mistakes to Avoid


  • Selecting a PII report when you intended a NON-PII report (for example, Payment schedule - overall PII report vs Payment schedule - overall NON-PII report). 

  • Skipping SELECT THE ROLE or choosing the wrong role for the report you’re adding. 

  • Using an application submission report when you actually need Forms non-submitted apps to see started-but-not-submitted applications.


Best Practices for FORWARD Direct Users


  • Use the report descriptions in SELECT THE REPORT to match the report to your purpose (PII, approvals, payments, or general application data). 

  • When adding reports, always select an appropriate option under SELECT THE ROLE (see User Roles)—the guide notes PII-Viewer is most common. 

  • For payment reporting, choose the option labeled NON-PII or PII based on whether you need PII elements included.
